Southend Reablement Service
The Complex Intervention Service provides a Reablement Service that is based on short-term intervention with an aim to get a person back to their base line to be able live safely within their home.
We provide a Reablement service that is led by a team of Therapist and create person centred plans that are focussed on outcome-based goals. We provide support to adults in their home, to engage with them to achieve the best outcomes possible by identifying aids, equipment, and exercise programmes to aid recovery and support the journey to independence This is achieved through mutual participation and engagement between the support worker and the adult. Through the Reablement journey we may signpost you to other agencies, who may be able to offer additional support once the Reablement service has ended. This may mean that through your journey you have been identified with the need to have further support, and this is discussed with you through regular reviews, and it may be necessary to refer you on for a long-term care package to manage your day-to-day care need.
Southeast Essex Enhanced Discharge Service (SEDS)
The aim of this service to support a discharge into your own home with the aim to assess your needs once in your own environment. This is led by a team of Therapist, within the NHS, who will aim to visit you with 72 hours of discharge and develop a support plan with you at the heart of it, and refer you on to either Reablement, Domiciliary, Care Home or discharge as Independent. This will be discussed with you by the Therapist that attend to complete the review with you at home.

Southend Reablement Service (SRS)
Reablement Service that is based on short-term intervention with an aim to get a person back to their base line to be able live safely within their home. We support you with daily tasks like personal care, dressing, meal prep but support is determined when the Care Supervisor will visit you in your own home when you are discharged, to build your care and support needs as your input is key to work towards your independence.
We would support with medication, if there has been a change to your prescription with the aim to support you to be able to manage this independently, but if family members have always sorted your medication, then under the Reablement Service we would ask them to maintain this. If you were then referred onto long term care (Domiciliary) then the service would be able to support with this if the medication were in the original packaging or blister packs
Under the Reablement service shopping and domestic tasks are not completed unless this is part of your care plan and goal to achieve this. We do have a private home care support service, Meadows Home Care, that can provide support in this area.
If you were referred onto a longer-term care package this may be built into your care plan with that provider. A Social would visit to to complete a Care Act assessment as this will form part of your package or care based on your needs. They would also go through a financial assessment with you to determine if your care would be funded by Social Care part funded or you would need to pay full costs. This is shared with you by a Social Worker as we do not complete this part of your assessment. If the longer term care package is private, we would be happy to discuss with you about your needs, as part of our Meadows Home Care Service
From hospital you would be referred to us under the Enhanced discharge service, this service should be for up to only 2 weeks.
Within this time your will undertake an assessment in your home from a therapist within the NHS and they would decide based on their findings, and this could be 1 of 3 pathways – Reablement, Domiciliary or end care as Independent.
Reablement is up to 6 weeks, and any time through this once you have reached your goals the service could be ended. If you require longer term private care at home, we would be happy to discuss how our Meadows Home Care service could support you.
Through the reablement service you will be reviewed a minimum of every 2 weeks. This will involve looking at your care needs to establish how you are progressing against your goals, through discussions with the supervisor and yourself.
We will look to see around the 4 week mark if you may need on-going support at the end of the Reablement service. If so we would refer to to our Care Broker who will then assign a social worker to contact you to discuss your care needs moving forward. Care will stay in place until a new provider is sourced for you, however we are also able to offer a private care at home service, through our Meadows Home Care service.
Under our reablement service we would be able to refer you to our Reablement Therapist who would visit with you to identify any equipment you may need and to order. At this stage there is no cost to you but if there was with a particular piece of equipment that wanted then this may incur a charge. This will be guided by the therapist and they would be able to advise you of this.
Under our reablement service we would be able to refer you to our Reablement Therapist, this includes a Physiotherapist. They would work with you on suitable exercises for you to complete and there would be no charge for this service
Complex Intervention Service is not a gender specific service, and we employ both male and female carers. Whilst we will do our best to support your choice we cannot guarantee this.
Your quantity of care visits would depend on your care needs and what is discussed when you are discharged home. On your return home you would have a care assessment with a Care Supervisor, and this is when it would be determined the care visits that are needed with your involvement with this process.
Under the reablement service there are not set times as we are encouraging independence. Within Domiciliary while the visits are not set time they are within a 2-hour slot where possible and this time slot would be shared with you.
If you choose to pay for your Care under our Meadows Home Care provision, then yes you would get a set visit time within a 30 min leeway.

Complex Intervention Service (CIS)
If your referred through the Enhanced Discharge Service or Reablement then care is funded by Health and Social Care and no cost will be passed on to you. If you identified for on-going care then you will be asked to complete a financial assessment form, and the outcome will be shared with you via a social worker.
Of course! We encourage family to be involved if you were happy with this. We are led by you with who you want information shared with. At the start of your journey with us we would ask you to sign a consent form. This would be consenting us to contact GP and other medical professionals in relation to you and your health and care needs.
You! You are involved with setting up your goals and care needs with the Care Supervisors. Other medication professionals can be involved so the best outcomes can be achieved, these can include Therapist, District Nurses, GP but not inclusive.
Of course you can, we encourage family outings as this is good for your mental health and wellbeing, all we ask is that you call us on 01702 215941 between 7am and 10 pm to give us notice that you don't need the call. This would save a wasted visit by one of our care team. Calls can be cancelled for up to 3 days, after this time the care package would be cancelled, and you would need to be re-referred. Meet The Team

Deborah started her journey in care 9 years ago after caring for her Nan. Deborah could see that good care can make a difference to someone whether it be in a Care Home or their home. Deborah knew after doing this she wanted to be able to support other people to live a fulfilling life.
Deborah started as a carer and enjoyed being able to support people to be as independent as possible and wanted to progress. This was so she could help others come into this role and make a difference. Deborah is now a manager of the Community Service at Southend Care and is fully focussed on helping people on this journey.
Deborah enjoys her role and the ability to support both a care team and individuals. Deborah feels if she can make a difference to one person a day, she knows she is doing a good job. She is passionate about delivering care centred on a person's individual needs and being able to support them to remain in their own homes. Having worked within a Reablement Service and then a Domiciliary Service, she has seen how both can have a positive impact on someone’s Mental and Physical Health.
In Deborah’s spare time she enjoys having friends and family over and enjoy cooking for them, this could be a BBQ or a meal. Deborah likes to see smiling faces and people having a good time. Her family are my greatest support she enjoys spending quality time with them.
Deborah’s other pastime is travelling, over the last few years she has been fortunate to visit places like St Lucia, Jamaica, and other destinations in the Caribbean and will continue to explore the world on these travels and visit new countries and cities.

Paula has worked in Social Care since 1993, started working in the community promoting rehabilitation and enjoying working alongside the customers, building their confidence and independence.
In 2006 she was promoted to an Assistant Manager where she remains in this post, Paula has been lucky to see all the great changes made throughout the years and feels proud that she has been a part of these changes and seeing these be a success.
Seeing the service through the 2022 CQC inspection and receiving a good rating has been Paula’s great achievement. She is passionate about supporting the staff to develop in their roles and ensuring all have a person-centred approach when delivering care. Paula enjoys spending time with her family, her fur babies and entertaining friends’ home and away. She is a dab hand at DIY and when the mood takes her she bakes a good cake.
